Insurance-based: Priority Health, BCBS/BCN of Michigan and Medicare.
***If you have insurance that is not listed above, we can still see you. This would require self-pay at the time of your appointment, to include a 10% discount. We will provide you with the necessary paperwork to submit a claim to your insurance company for reimbursement.
Best for those who have schedule flexibility for more frequent in-person follow-up visits; medicare must use this model

(depending upon time + complexity spent on evaluation & treatment plan)
• Follow-up visits: 15-30 minutes
o $106-280

• responsible for copay and whatever insurance doesn’t cover
o Check with your insurance about registered dietitian / diabetes educator coverage
• Registered Dietitian visits are $65 for 30 minutes and $120 for 60 minutes one-on-one
Direct Fee-For-Service:
HSA/FSA can be used. Out-of-network insurance must use this option.
Best for those who have financial flexibility and desire longer visits and less frequent in-person visits.
